Waikato DHB Accepts Coroner’s Findings

Waikato District Health Board has today accepted the findings of coroner Gordon Matenga into the death of baby Adam Barlow at Waikato Hospital on 25 October 2009.

Chief operating officer Jan Adams said she was pleased the coroner found Waikato DHB’s serious event review process to be robust.

“I would like to offer Linda, Robert and Orry Barlow our heartfelt sympathy on behalf of all the staff at Waikato DHB on the loss of their son and brother, Adam.

“The coroner accepted the findings of our internal investigation, which was carried out as part of the national serious and sentinel event reporting process.

“Our own internal investigation did find some areas we needed to improve on and the coroner noted that,” she said.

“Many of the recommendations made by the coroner have already been actioned by the DHB and the DHB will work to ensure that that we continually improve the services that we offer to our patients.”

Waikato Hospital clinical director obstetrics Dr Penelope Makepeace said she accepted the coroner’s recommendation that a registrar consult with the obstetric consultant for every case where there has been a transfer.

“We accept we need to support junior staff and are working with them as to how that will happen,” she said.

“We also note that new referral guidelines have given greater clarity in how transfers should be made and I welcome the incorporation of emergency management transfer guidelines.”
