ProCare leads the world winning ‘Best International Chronic Disease Management Programme’

ProCare Health’s chronic disease management programme has been named the ‘Best International Chronic Disease Management Programme’ at the Global Perspectives on Chronic Disease Prevention and Management Conference held in Calgary from October 29 to November 1.

ProCare was the only international organisation recognised by the inaugural awards, which acknowledged high-level achievement for chronic disease prevention and management.

Paul Roseman, ProCare’s Design and Development Manager, attended the conference to present a paper on ProCare’s Chronic Disease Management programme, which is available in ProCare general practices. He was delighted to accept the award and trophy on ProCare’s behalf.

“The recognition that ProCare is a leader in the world of chronic disease management demonstrates that what we are doing here in New Zealand is pushing the boundaries and achieving some very real results for the health of the 650,000 Aucklanders we care for,” Paul Roseman says.

The scientific judging panel particularly commended us on our programme’s completeness, its incorporation of technology, the way we have integrated it into our communities and the engagement with health care providers.”

Paul Roseman’s attendance at the awards was supported by NZ Trade and Enterprise and software developer, Enigma Publishing.

About ProCare

ProCare Health Limited represents about 500 independent General Practitioners (GPs) throughout the greater Auckland region in New Zealand. Depending on where their practices are situated, the GPs are aligned to one of ProCare’s three primary health organisations (PHOs). These are: ProCare Network North (aligned to the Waitemata District Health Board), ProCare Network Auckland (aligned to the Auckland District Health Board) and ProCare Network Manukau (aligned to the Counties Manukau District Health Board).

Each of the three ProCare PHOs has an independent board of directors. The PHOs consult within their communities to ensure they provide primary health services that are appropriate to local needs. They negotiate with the District Health Boards for funding of these services.
